It covers this website only. How we handle your health information as a patient in our office is described in our separate Notice of Privacy Practices, explained below.

Information you give us

When you send us a message through the contact form on this website, we collect your first name, last name, email address, phone number and the message you write. We use this information only to respond to you, for example to answer a question or help you schedule a visit.

Messages sent through the form are delivered to our office by email and a copy is saved in our website’s system. Please do not include detailed medical information, insurance numbers or other sensitive health details in the form. If you need to share health information, call our office and we will help you directly.

Information collected automatically

Like most websites, ours collects some information automatically when you visit:

  • Google Analytics. We use Google Analytics to understand how visitors use our website, such as which pages are viewed, how long visits last, the general location (city or region), the device and browser used, and how visitors found our site. Google Analytics uses cookies to do this. We use this information to improve our website.

We keep contact form messages for as long as needed to respond to you and for our normal business records, and then delete them. Analytics information is kept according to Google Analytics settings.

How we protect information

We use reasonable measures to protect the information collected through this website, including a secure (encrypted) connection. No website or online transmission is completely secure, so please avoid sending sensitive health information through the website.

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from children through it. A parent or guardian should contact our office directly about a child’s care.
